Participant Requirements

 

• Roxbury resident
• Sixty (60) years of age or over
• Disabled/Handicapped – Eighteen (18) years of age or over
• You must have a companion if your condition requires you to be assisted

 

All reservations must be made at least 24 hrs in advance and not more than 5 working days in advance.

 

Once your appointment has been made, THERE IS NO NEED TO CALL AND CONFIRM YOUR APPOINTMENT.

 

Services Provided

 

·        Monday          Rockaway Mall -- Medicals

·        Tuesday         Roxbury Mall -- Medicals

·        Wednesday   Roxbury & Ledgewood Malls -- Nutrition Site -- No Medicals

·        Thursday        Senior Club Meetings -- Medicals

·        Friday             Nutrition Site – Medicals

 

At your request Dial-A-Ride guidelines will be sent to you, along with a registration card.  If you decide to participate please return it to the health department completed.

 

If you are handicapped or disabled and not age sixty (60) or older, it is required that your doctor fill out a certification form, these forms are available through the health department.

 

Once the health department has received the completed registration and/or certification form, your name will be added to the dial-a-ride passenger list, and you can begin utilizing the services.
